Practical training in difficult airways management

The medical faculty of V.N.Karazin Kharkiv National University hosted practical training for healthcare professionals. The event took place on 12th and 13th of February 2021. “It is the very first “Difficult airways” course based on Difficult airways society guidelines which took place in Ukraine.”- comments Andrii Telezhnyi, the head of Karazin medical simulation center: “The course had not been conducted previously. Mainly due to the absence of infrastructure and proper equipment. However thanks to the cooperation of the instructor teams from Kharkiv and Dnipro cities we have been able to conduct this event successfully.”

The course organization was sufficiently supported by the administration of medical faculty and Karazin university.

The mentor of the course - Denis Surkov, MD, anesthesiologist, the head of neonatal intensive care unit at the Dnipropetrovsk Oblast Children's Clinical Hospital, Associate Professor at the Department of Disaster Medicine and Military Medicine of the Dnipro Medical Academy of The Ministry of Health of Ukraine.

The course was composed of a theoretical part and practical training with the use of high-fidelity manikins and equipped with world’s leading manufacturers Gaumard (USA), Ambu (Denmark), BronchMentor Simbionix 3D Systems simulator (Israel), etc.

The instructors group not only was made up of medical faculty teachers and medical simulation center workers but also included highly qualified professionals from different fields of medicine. The instructor group consisted of specialists from Kharkiv and Dnipro cities. The heads of departments, anesthesiologists, emergency physicians, EMS professionals became the participants of the course such as professionals from Kyiv, Poltava, Zaporizhzhia, Dnipro, and Kharkiv.

The majority of course participants as well as the instructor group consisted of acting European Resuscitation Council (ERC) instructors.

During the course the following theoretical and practical skills were issued:

-Difficult intubation risks assessment

-Rapid sequence intubation (RSI)

-Supraglottic airway devices

-Bogie intubation

-Fiberoptic intubation

-Ultrasound for the management of the difficult airways

-Apneic oxygenation, High Flow Nasal Cannula

-Surgical airway, cricothyrotomy

-Protocol for complicated bag valve and mask ventilation during induction, unanticipated difficult tracheal Intubation during induction, “can’t intubate / can’t ventilate”.

The course is going to be a regular event that will be held monthly at the Karazin University. “School of emergency care” project is the next planned step. The school will be created under the auspices of the Ministry of Health of Ukraine and in collaboration with the NGO’s “All-Ukrainian resuscitation council” and Surgical club “Cor Fortis”. The airway management course will be the mandatory part of the future program.
